Start/End Date: Select the date range for the report.

Procedure Code: Select a specific Procedure code or type ALL to pull report for all Procedure codes.

Click Go to run the report.

This report subtotals by Procedure code and will show the patients names, date of Service and Amount, as well as the number of times the Procedure code was used between the date range; for a more detailed review of each Procedure code.
